The simplest way to make a referral is to call our main line at (669) 201-4949 to speak directly with us. If you leave a message, one of our therapists will return your call as soon as possible to discuss any questions or queries you may have.
To make a referral via e-mail, simply send us a secure message at AskUs@liffeyhealth.com or use the Contact Us page on this website. Please use our Referral Form to provide the appropriate consent information for your client. If you have yet to receive consent, please do not include any identifying client information.
To make a referral via fax, simply fax a copy of our referral form to (669) 295-1513 and we will respond to your query within 24 hours.
